ORDERING INSTRUCTIONS:
Download and print a copy of the order form.  Fill out form completely.
Names will can be added to the duffel bags, sweatshirts and skull caps at no extra charge. Include the name you would like on each item on the order. Please print clearly. Order forms and checks are on or before July 29th at the equipment handout.

Note: Orders placed for the July 29th due date will most likely not be in by the time the season starts.

Orders will not be placed without payment.

Orders with payment must be received by April 30, 2010. This will insure delivery by July.

Ordered apparel will be brought to practice for pick up.
